- DO NOT ABBREVIATE - Write all entries in full
- Use the full name of your cadre, rank, and station
- Select your hospital from the dropdown list
- If your cadre is not listed, click on "➕ Other (specify)" at the bottom of the cadre dropdown to enter your cadre manually
- Medical Officers, Dental Officers, and Pharmacists must indicate if they are Consultants
- Select your highest qualification from the dropdown (this is not needed for Doctors, Dental Officers, and Pharmacists)
- Ensure all dates are correct
